How to Choose an Omega-3 Supplement: A Practical Buying Guide
This guide explains how to read an omega-3 label, what the research actually supports, and how to tell a well-made supplement from clever marketing — so you can compare products on your own.

What Is Omega-3?
FACT: Omega-3 fatty acids are a family of polyunsaturated fats. The three most relevant to supplements are ALA (alpha-linolenic acid, found in plant oils), EPA (eicosapentaenoic acid) and DHA (docosahexaenoic acid), the two marine forms found in fish and algae. The human body can only convert a small fraction of ALA into EPA/DHA, which is why direct dietary or supplemental EPA/DHA matters.
EVIDENCE: EPA and DHA are incorporated into cell membranes and support normal cardiovascular and cognitive function; these are reflected in nutrient reference intakes set by bodies such as EFSA and the U.S. NIH, and in qualified health claims recognized by the U.S. FDA (1, 2, 6). UNCERTAINTY: Most clinical research has focused on specific populations and outcomes (heart health, triglycerides, pregnancy), and effects in healthy individuals vary — supplements are not a substitute for a balanced diet, and recent meta-analyses report small or uncertain effects on cardiovascular outcomes in general populations (8, 9).

How to Read the Label
The most common marketing trick is printing a big number (e.g. “1,000 mg fish oil”) on the front while the actual EPA + DHA content hides in the supplement facts panel. Here is what to check, in order:
- Serving size — usually 1 or 2 softgels. All numbers below are per serving, not per bottle.
- EPA and DHA listed separately — add them. This is your real dose.
- Total oil weight vs EPA+DHA — “1,000 mg fish oil” can contain as little as 300 mg of EPA+DHA. The rest is filler oil.
- Compare across products by EPA+DHA per serving, then by price per gram of EPA+DHA.
If a label does not state EPA and DHA amounts separately, you cannot evaluate the product — treat that as a red flag.
Forms of Omega-3: TG, rTG, EE and Algal Oil
Fish oil comes in different chemical forms, and marketing loves to make this sound more important than it is:
- TG (triglyceride form) — the natural form found in fish.
- rTG (re-esterified triglyceride) — processed back into triglyceride form, generally with higher EPA+DHA concentration per capsule.
- EE (ethyl ester) — the cheaper, concentrated form used in many value products.
- Algal oil — a plant-based source of DHA (and sometimes EPA), suitable for vegetarians and vegans.
EVIDENCE: Short-term studies have reported somewhat better absorption of TG/rTG forms than EE (3). UNCERTAINTY: The clinical significance of this difference over months of daily use is not well established. A good dose and verified purity matter more than the form on the label. We do not recommend paying a large premium for rTG alone.
Third-Party Testing: IFOS, USP, NSF and COA
Third-party testing verifies what is actually in the bottle: contaminant levels (heavy metals, PCBs), oxidation state, and whether the label dose is accurate. What the common marks mean:

FACT: These certifications confirm quality control, not health outcomes (4, 5). A product without any published testing information is a risk you cannot evaluate — that alone is a reason to keep looking.
How Much Do You Need?
EVIDENCE (educational ranges only): International bodies suggest a daily EPA+DHA intake in the range of roughly 250–1,000 mg for general health, with specific recommendations varying by age, pregnancy status and health context (1, 2). During pregnancy, DHA intakes around 200 mg per day are commonly recommended by professional organizations (7).

What to Avoid
- Labels that quote fish oil weight instead of EPA+DHA — you cannot compare these products.

- Fishy smell, burp-back or rancid taste — possible signs of oxidation; freshness matters for safety and tolerability (4).
- Therapeutic marketing language (“helps cure”, “treats”, “prevents”) — reputable brands do not make such claims about supplements.
- Huge bottles at suspicious prices — calculate price per gram of EPA+DHA before you buy.

FAQ
Does fish oil cause fishy burps?
Some people experience it, more often with oxidized or low-quality oils or when taken on an empty stomach. Enteric-coated capsules and taking it with food can help. Persistent burp-back can also be a freshness warning.
I don’t eat fish — what are my options?
Algal oil supplements provide DHA (and some EPA) from a plant source and avoid the fishy taste and marine-sourcing concerns. ALA-rich foods (flaxseed, chia, walnuts) are useful but convert to EPA/DHA only in limited amounts.
Does the EPA:DHA ratio matter?
For general health, total EPA+DHA matters more than a specific ratio. Certain life stages (e.g. pregnancy) emphasize DHA. Evidence on ideal ratios for healthy people is limited.
Can I take omega-3 with blood thinners?
High-dose omega-3 can affect blood clotting. If you take anticoagulant or antiplatelet medication, have a bleeding disorder, or are facing surgery, discuss supplementation with your clinician first.
